Question about a VARIABLE ANNUITY DEATH CLAIM FORM? - annuity death
I am currently completing the forms to confirm the death of my grandparents gave me a pension. This is the documentation that keeps up with the death certificate to the financial institution the funds originated. My question is, once all information MART * IE * Social Security personal # 's and dates of birth, you signed the "applicant's signature" no place for a signature "cookie". Is it necessary to have it signed by a lawyer or someone to sign as a witness? Thank you.
